The July issue of Peritoneal Dialysis International (PDI) features the latest update of the ISPD recommendations for treatment of peritoneal dialysis (PD)-related infections. ISPD recommendations for peritonitis and exit-site infection were first published in 1983 and can arguably make the claim to be the first set of “Clinical Practice Guidelines” in nephrology. This 2010 update is the seventh version. Undoubtedly, these are the best known and most important clinical practice recommendations produced by the ISPD and their influence is large. It is notable that, while many national nephrology societies have produced guidelines or recommendations on other areas of PD, almost all have not addressed peritonitis and exit-site infection but have left this to the expertise of the ISPD Committee.

Also in this issue of PDI are the new Guidelines on Peritoneal Access, drawn up last year by an international committee of experts chaired by PDI Associate Editor Martin Wilkie under the auspices of the United Kingdom Renal Association. These guidelines have been endorsed by the ISPD and are now in print for the first time. They replace the 2005 update authored by Michael Flanigan and Ram Gokal. They provide a comprehensive guide to how peritoneal catheter insertion should be organized, carried out, and audited. They recognize the variety of methods used to insert catheters and, rather than endorse one over another, they focus on the more critical issues of local expertise and monitoring of outcomes. They lay down standards that need to be met in a high quality PD program.
